Long Term Care
Access to Long Term Care is coordinated through the Community Care Access Centre. Residents in this level of care receive comprehensive nursing and medical assessments on admission and quarterly. Each day, residents receive on average 2.7 hours of nursing and personal care.
We provide specialized home environments, depending on care needs. Some home areas are specifically designed for those requiring physical assistance with activities of daily living, and some are specifically designed for those with Alzheimers Disease or dementia. Separate home areas are available for those in the active stages of the disease and for those in more advanced stages so their care can be tailored to meet their personal needs. There is also a home area for behavioural support as well as home areas for persons whose care needs are primarily physical in nature, including young adults.
Our Long Term Care Homes are licensed by the Ministry of Health and Long-Term Care and funded through the Local Health Integration Network (LHIN) via the Long-Term Care Home Service Accountability Agreement (L-SAA). Click here to view a copy of the agreement (this may take a few moments to download). Access to this level of care is coordinated through the Community Care Access Centre (CCAC). Find your local CCAC at www.ccac-ont.ca.
Long Term Care is available at all of our locations.
