Specialized support within long-term care

At Schlegel Villages, we believe everyone deserves to live with dignity, comfort, and belonging. The ESN is a transitional neighbourhood within the long-term care home at The Village of Ridgeview Court in Milton, that provides specialized, relationship-centred support for residents living with dementia who require the insights of the Schlegel Villages philosophy to help find the rhythm of care and support best suited to their unique needs.

The Enhanced Supportive Neighbourhood is grounded in our Living in My Today philosophy, a social model of living that focuses on understanding the person.
Personal expressions are understood as communication. Our team responds by adapting relationships, routines and environments so residents can experience meaningful days, renewed connections, and the freedom to live well. 

Families often tell us that when the right rhythm of care is found, their loved ones begin to look and feel more like themselves again. That is the heart of this work. 

Designed to Support Comfort and Stability

Residents in the Enhanced Supportive Neighbourhood live in a thoughtfully designed neighbourhood that reflects the warmth and familiarity of home. These supports allow team members to spend more time building relationships and responding early to changes in need.
The goal is not simply to create a calm environment, but to create the conditions in which residents can flourish.

Part of One Village Community

The Enhanced Supportive Neighbourhood is part of our Village community. While it offers enhanced supports for some residents, the Village remains one unified home where daily life across neighbourhoods continues with shared values of dignity, belonging, and mutual respect.

Care Planning and Transitions

Care in the Enhanced Supportive Neighbourhood is guided by knowing each resident deeply – through their story, routines, strengths, and relationships. Families and substitute decision-makers are valued partners and are involved in care planning and ongoing conversations.
The ESN is transitional in nature. Its purpose is to help residents rediscover a sustainable rhythm of care that enables a successful transition into a long-stay long-term care home, when appropriate.
Length of stay is not fixed and is based on each resident’s needs. Many residents remain for several months while relationships are built, approaches are refined, and wellbeing is strengthened.
For many residents, the Enhanced Supportive Neighbourhood supports a particular stage of their journey. Transition planning begins early and is done thoughtfully to support dignity, continuity, and stability.

How to Access the Enhanced Supportive Neighbourhood

The Enhanced Supportive Neighbourhood is considered as Behavioural Support Transitional Unit and admissions and placements are coordinated through Ontario Health atHome, based on individual need and readiness for this level of support. Our team works closely with Ontario Health atHome, hospitals, and families to support clear communication and smooth transitions.
